Reimbursement Information

If you are receiving support from us you will have to fill in the Reimbursement Information Form and submit it together with your original receipts and other required documentation to Crystal N. Davis (see the contact information below). If you send your documentation by email, CC to Anna Skripka.

Legal permanent resident and US citizen will also have to provide:
1. a completed and signed substitute W-9 form
2. a copy of your US Green Card/Resident Alien Card (if not a citizen)

Foreign nationals that are being reimbursed will also have to provide:
1. a completed and signed W-8BEN form
2. a copy of the oval HomeLand Security stamp dated for this visit to US
3. a copy of the passport front page with the picture/country information
4. if not from a visa-waiver country, a copy of the visa for the current visit to US
5. if on H1B/J1/F1 visa, a copy of I-797C, IAP66/DS-2019, or I-20 (all pages), respectively, as well as a letter from the designated officer stating that the visitor is authorized to participate in the activity at UNM and that UNM is authorized to make travel reimbursement directly to the visitor [sample letter]
